The application form is FLR(M) that is correct - your wife will need a separate appointment to make her application.
If you apply using Category B you need to evidence that you have earned the minimum of £18,600 in the 12 months prior to the application being submitted.
You must also evidence that you will earn the minimum of £18,600 going forward - based on the figures you have stated you are showing that you will earn £16,143.40 per year this would not meet the requirements of the immigration rules.

The English test can be taken whenever your partner feels ready as this is valid for 2 years from the date of issue. There is no requirement for the documents to be in a particular order when submitting them to the Home Office.
Provided you met the financial requirements at the time of submission there is no need to update them on your pay rise.
The Home Office request to see the original documents prior to scanning in order to asses any forgery if the deem necessary.
You are not required to evidence your employment status it is just a question on the application form. If you husband does send you money then do provide this as evidence to support the application. If not do not worry.
